How to Request Your Health Care Records
If you would like to obtain a copy of your medical records, please follow these steps:
1. Visit Our Facility:
Come to our location in person. We require that all requests be made onsite to protect your privacy and ensure proper verification.
2. Bring Your Identification:
Please bring a valid driver’s license or another government-issued photo ID. This helps us confirm your identity before releasing any medical information.
3. Complete a Medical Release Form:
At the facility, you will need to fill out and sign a medical records release form. This form authorizes the release of your health information and ensures compliance with privacy laws.
4. Receive Your Records:
Once the form is completed, a copy of your medical records will be printed and provided to you onsite.
